Covid Clinic's Rapid COVID-19 Antigen Test:

Covid Clinic’s nasal swab tests are used to detect the presence of COVID-19. The tip of a cotton swab is inserted into both nostrils. The swab is rotated in a circle around the entire inside edge of each nostril at least three times.
Potential risks include:
- (i) possible discomfort or other complications that can happen during sample collection; and
- (ii) possible incorrect test result.
Potential benefits include:
- (i) the results, along with other information can help your healthcare provider make informed recommendations about your care; and
- (ii) the results of this test may help limit the spread of COVID-19 to your family and others in your community.
Upon obtaining the sample, it will be processed on site and the results will be provided to you, usually within an hour.
The Rapid COVID-19 Antigen Test used by Covid Clinic has not been FDA cleared or approved. The test has been authorized by the FDA under an EUA for use by authorized laboratories. This test has been authorized only for the detection of proteins from SARS-CoV-2, not for any other viruses or pathogens. This test is only authorized for the duration of the declaration that circumstances exist justifying the authorization of emergency use of in vitro diagnostics for detection and/or diagnosis of the virus that causes COVID-19 under Section 564(b)(1) of the Act, 21 U.S.C. § 360bbb-3(b)(1), unless the authorization is terminated or revoked sooner.
I also understand that Covid Clinic participates in clinical research activities and that I may be asked to participate in these research activities. I further understand that my participation is 100% voluntary and that the care I receive from Covid Clinic will not be impacted by my decision as to whether or not to participate in such clinical research activities.
